PHOTO RELEASE

 

For good and valuable consideration, the receipt of which is herby acknowledged, I, the undersigned, hereby grant ___Hearts for Healing, Inc______ permission to use my likeness in photography in any and all of it’s publications, including but not limited to all ___Hearts for Healing, Inc______ printed and digital publications. I understand and agree that any photograph using my likeness will become property of ___Hearts for Healing, Inc______ and will not be returned.

 

I acknowledge that since my participation with ___Hearts for Healing, Inc______ is Voluntary, I will receive no financial compensation.

 

I hereby irrevocably authorize ___Hearts for Healing, Inc______ to edit, alter, copy, exhibit, publish or distribute photos of myself for purposes of publicizing ___Hearts for Healing, Inc_____ program or for any other related, lawful purposes. I waive the right to inspect or approve the finished product, including written or electronic  copies in which my likeness appears. I waive any right to royalties or other compensation arising out of or related to the use of the photograph.

 

I hereby hold harmless and release and forever discharge ____Hearts for Healing, Inc_____ from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators, or any other person acting on my behalf or on behalf of my estate have or may have reason of this authorization.

 

I am at least 18 years of age and competent to contract in my own name.  I have read this release before signing below with the intent of it being legally binding and fully understand the contents, meaning and impact of this release.
